Isaac Clarke is an ordinary engineer on a mission to repair the USG Ishimura, a massive mining spaceship, only to discover that something has gone terribly wrong. The ship's crew has been massacred, and his beloved partner, Nicole, is missing somewhere on board.

Now alone and armed only with his engineering tools and skills, Isaac rushes to find Nicole as the nightmarish mystery of what happened aboard the Ishimura unfolds. Trapped with hostile creatures called Necromorphs, Isaac faces a fight for survival not only against the ship's growing horrors but also against his own deteriorating sanity.
